Enhanced Care Management
Your Personal Care Navigator
ECM is a benefit providing dedicated support for individuals with complex health and social needs – at no cost to eligible members.
1️⃣ Check Eligibility – Medi-Cal members with complex needs, housing challenges, serious mental health needs or those involved with foster care may qualify
2️⃣ Meet Your Advocate – You’ll be connected to a lead care manager who will be your single point of contact
3️⃣ Access Your Care – Your care team coordinates physical, behavioral, dental and social services – in your community where you live
