2020 Vision Youth is an official provider of community health worker services through Molina Healthcare, Kaiser Healthcare, Independent Living Systems, IEHP and Medi-Cal healthcare.

Community Health Work (CHW)

Coordinating care
● Connect individuals with healthcare providers and social service agencies
● Make sure individuals get the necessary care and services they are
needing/requesting
● Help individuals with communication barriers/gaps especially with the healthcare
system
● Assist individuals with navigating their resources as well as their systems of care
and what’s available to them
● Advocate for those individuals and make sure the receive the services that are
necessary for them

Health Education and Outreach
● Help healthcare advise fit the cultural needs of the community as well as the
individuals needs
● Help engage the community members to raise awareness about community needs
● Help the community members make better lifestyle choices
● Promote lifestyle changes;encourage and support individuals and also help with
adopting positive health habits

Case management and resource access
● Help in assisting individuals in navigating the healthcare system
● Connect people with social services (housing, food assistance,education,social
support,financial assistance, etc.)
● support individuals in obtaining financial assistance benefits
Advocacy
● CHWs should advocate for the individuals and the communities
● Make sure that our clients are heard in the decision making process
● Promote equitable access to services and resources as necessary

Data collection and reporting
● Collect data on community health needs
● Collecting data identifies the trend of issues affecting the community’s well being
● Share this information with health care providers
● This gives us a way to expand resources to be able to fit everyone’s needs

Collaboration
● Our company will collaborate with healthcare providers (IEHP,Molina,Kaiser..Etc.)
● Engage with other collaborators to support the community’s healthcare needs
● Achieving goals by reaching out to other to partner/help

Among the many known outcomes of CHWs’ service are the following:
