This year, we have scaled our structural reach by integrating Certified Community Health Workers (CHW’s)[cite: 1]. As trusted public health advocates operating directly inside the community, our CHW’s break down barriers to care by linking families with housing, food banks, utility relief, transit support, and career education[cite: 1].

We are now official asset providers for IEHP, Molina, and Kaiser Permanente, allowing our network to provide care configurations across the Inland Empire without limitations[cite: 1].
