This year, we have scaled our structural reach by integrating Certified Community Health Workers (CHW’s). As trusted public health advocates operating directly inside the community, our CHW’s break down barriers to care by linking families with housing, food banks, utility relief, transit support, and career education.

We are now official asset providers for IEHP, Molina, and Kaiser Permanente, allowing our network to provide care configurations across the Inland Empire without limitations.
